Counseling Consent Form
Dear Esteemed client,
To establish a mutual understanding of the rights and obligations of both you and our therapist, we refer you to our terms and conditions prior to the commencement of the counseling sessions. By consenting to the contract, you agree to fulfill your duties as outlined in the contract.
How it works
Each counseling session at Ankodi Relationship Therapy will have a duration of 45 minutes. The number of sessions can be extended after regular reviews of the counseling work with the therapist. It's important to note that during the sessions, it is not upon the therapist to provide solutions, rather support you to come up with solutions that addresses your situation.
1. Place of Session
Ankodi Relationship Therapy will provide counseling sessions in both in-person and online formats while maintaining high levels of privacy and confidentiality. The in-person sessions will be held at our office located at Diani Close, Oledume Road, Nairobi or at a location designated by the therapist. The online sessions will be available as an alternative option.

During counseling sessions, you can also contact the therapist using their respective telephone number or email address. It's important to note that voicemails cannot be left on the therapist's phone, so if you are unable to reach them via phone, please send an email or use another contact method.
3. Boundaries of Contact
To ensure effective communication, we will communicate the therapist's office hours and contact information to you. In the event of an emergency, please follow the emergency protocols specified by the therapist. It's important to note that if the therapist is not available to respond immediately, you should seek assistance from the appropriate emergency services.
4. Rescheduling and Cancelation
If you need to reschedule an upcoming appointment with Ankodi Relationship Therapy, you can contact us at info@ankodi.org. Please also let us know how you prefer to be contacted in this situation.
Additionally, if you're running late for your appointment, please inform us and indicate whether you would like us to contact you and your preferred contact method. To cancel or reschedule your appointment, it is required that you provide at least 24 hours’ notice for cancellation and 48 hours’ notice for rescheduling, especially if you cannot attend your appointment.
5. Payment Schedule
The client is required to make FULL PAYMENT for the initial session in advance based on the category rate and a 50% deposit of the summation of the subsequent sessions. To avoid TERMINATION amidst therapy.
6. Complaints
If you are dissatisfied with any aspect of the service you receive, you should be aware that there is a formal complaints procedure available. This agreement is fully understood and agreed to and stands as a contract for our counseling work together.
